Reliance Lab Testing Solutions, LLC provides professional testing services, including but not limited to:

All services are provided in accordance with applicable laws and regulations.
2. Eligibility
By using our services, you represent that you are:

At least 18 years old (or accompanied by a legal guardian if under 18).

Authorized to request and receive the testing services.

Providing accurate, truthful, and complete information.
3. Payment & Fees

All fees for services must be paid in full at the time of service unless prior arrangements are made. Deposit is required at time of booking.

We accept cash and credit/debit card.

Payments are non-refundable once services have been rendered, except as required by law.
4. Test Results & Reporting

Results will only be released to the tested individual, their legal guardian, or an authorized representative.

For legally admissible tests (e.g., court-ordered DNA testing), results will be handled in compliance with chain-of-custody requirements.

We are not responsible for how results are interpreted, used, or disclosed by third parties once delivered.
5. Confidentiality

All test results and personal information are treated as confidential.

We comply with applicable federal and state privacy regulations, including HIPAA.

Results will not be shared with unauthorized parties unless required by law.
6. Client Responsibilities
By using our services, you agree to:

7. Limitations of Liability

We strive for accuracy in all testing; however, Reliance Lab Testing Solutions, LLC is not liable for damages arising from:

Delays caused by third-party laboratories, courier services, or unforeseen events.

Errors resulting from false, misleading, or incomplete information provided by the client.

Our maximum liability is limited to the cost of the service provided.
8. No Medical Advice

Our services are for informational and legal purposes only.

We do not provide medical diagnosis, treatment, or medical advice.

Clients should consult a licensed healthcare provider for medical concerns.
9. Cancellation & Rescheduling

Appointments may be canceled or rescheduled with at least 24 hours notice.

Failure to appear without notice may result in a cancellation fee.
10. Intellectual Property
